Hospital Liaison
Description
As a Clinical Liaison, you will serve as the primary point of contact between new and existing patients of Southeast and area psychiatric hospitals. You will facilitate continued treatment to clients of Southeast while they are hospitalized at private psychiatric hospitals, as well as assessing new clients for admission to our community treatment teams.
Key Responsibilities:
Meet with patients in the hospital setting to complete intake assessment and team assignment.
Advocate on behalf of clients to facilitate collaborative discharge planning between the hospital social worker and the agency case manager.
Facilitate the initial linkage meeting between the client and case manager while the client is still in the hospital.
Build and maintain positive relationships with hospitals, clinics, and other medical providers to ensure collaborative care. Unique benefits at Southeast:

BA/BS required; master's degree strongly preferred.
LSW/LPC required.
Strong communication and interpersonal skills, with the ability to build relationships with a diverse group of individuals.
Excellent organizational skills, with the ability to manage multiple priorities and deadlines.
Knowledge of healthcare systems and patient care processes.
Proficient in Microsoft Office Suite and electronic health record (EHR) systems.
