The Community Liaison/Prevention provides services to individuals, families, and groups on the south side of Columbus. This position is responsible for identifying therapy and case management needs, ensuring high-quality client care. The Liaison will also provide prevention, consultation, and educational services to schools, community groups, and agencies. The role involves assisting in the development and implementation of programming under the direction of the Program Director.
Key Responsibilities:
Provide individual, family, and group services to clients in the community.
Provide prevention and educational services to school constituents, community groups, and other local agencies.
Consult with schools, agencies, and community groups to develop initiatives aimed at improving community health and well-being.
Assist in the creation, planning, and implementation of community outreach programs in collaboration with the Program Director.
Act as a liaison between clients, families, and service providers to ensure proper coordination of care and support.
Develop and maintain relationships with community stakeholders to ensure the effective delivery of services.
Monitor and evaluate the effectiveness of outreach programs and suggest improvements where necessary.
Participate in community meetings and represent the agency at local events, providing information and resources to community members.
