Summary

The Facility Advanced Practice Provider oversees the process of assessment, diagnosis, and treatment of individuals with medical complaints or potential medical issues using his/her full scope of therapeutic skills, including the prescription of medication and nursing treatments. He/she will work efficiently to attain and maintain the highest level of care in accordance with regulatory standards. He/she also performs History & Physical Exams and Physical Evaluations as delegated by the Medical Director as well as performs a vital function in aiding clients on their road to recovery.

Responsibilities

Each provider is required to take calls for their region one day per week. 2 days per week if another provider is on PTO or out unexpectedly. Calls outside of normal business hours will route to the provider on call. Standard response time is 30 minutes

Direct patient care, including the use of prescription medications, whether through written or standing orders

Assess face to face all patients at all levels of care who have a valid medical complaint

Direct aftercare and discharge planning as needed

Maintain appropriate state licensure

Have no pending or past sanctions, debarments, suspensions, or exclusions by any federally funded healthcare program, including Medicare and Medicaid

Act in accordance with the policies, procedures, and standards of the Facility including all relevant regulations of JCAHO and State Regulations

Performs a thorough psychiatric evaluation in compliance with all relevant standards of the Joint Commission for the Accreditation of Healthcare Organizations (Joint Commission) within the first twenty-four (24) hours following a patient's admission to the Facility

Perform a thorough medical history and physical examination in compliance with all relevant standards of Joint Commission within the first twenty-four (24) hours following a patient's admission to the Facility

Assess face-to-face all patients at a Detoxification level of care, as defined by the Facility's Utilization Review department, 365 days per year

Assess face-to-face all patients at all levels of care other than Detoxification, as defined by the Facility's Utilization Review department, no less than once per week of their stay at the Facility

Attend all Treatment Team meetings and/or other appointments requested by the Facility to discuss the coordination of patient care across departments and to review patient progress toward stated goals and objectives of treatment Position Requirements

Qualifications

Must hold active Nurse Practitioner license

Must hold at least a master's degree in Nursing from an accredited institution of higher learning.

Minimum 5 years' experience in nursing in a mental health setting preferred

Prior experience in the chemical dependency field preferred

Minimum 2 years' experience in a managerial role preferred

Personal passion or interest in substance abuse issues strongly preferred

Must be committed to drug-free lifestyle and responsible alcohol use
