Job Description

Behavioral Health Specialist (LISW/LCSW) Responsibilities:

Performs psychosocial assessments, diagnoses, and provides brief therapeutic services to patients and their families.

Work closely with the entire clinical team as well as Behavioral Health Medical Director to manage complex patient cases and transitions of care.

Manage complex behavioral and psychosocial needs that result in improved clinical and program outcomes.

Monitors patient progress and adjusts patients’ treatment plan as needed.

Collaborates with the WellBe interdisciplinary team to develop a treatment approach consistent with the patients’ medical, social, and cultural needs.

Ensures compliance with regulatory agencies and WellBe clinical guidelines.

Effectively coordinates with community agencies and resources to meet the patients’ care goals.

Monitors and supports patients’ physical and emotional safety needs and follows mandatory reporting guidelines to ensure safety and compliance.

Work with the community team on quality measures, patient outcomes, and efforts to support high needs patients.

Maintains current written progress notes and other documentation on the member Medical Record including signed and dated documentation for all services in compliance with WellBe documentation policy.

Other tasks needed to accomplish the team’s objectives/goals.

Job Requirements

Behavioral Health Specialist (LISW/LCSW) Qualifications:

Master’s degree in social work

Minimum of 2 years post-Masters clinical social work experience within the behavioral health field.

Current unrestricted LISW/LCSW license in applicable state(s) required.

Have a valid driver’s license, automobile insurance, have access to an automobile; and proof of automobile insurance required.

Behavioral Health Specialist (LISW/LCSW) Skills We Value:

Competence in working with older adults in the community

Ability to complete psychosocial assessments, identify accurate behavioral health diagnoses, and implement appropriate treatment plans.

Must be familiar with EHR medical documentation

Strong computer skills, including programs within the Microsoft platform

Strong verbal and written communication skills

Competence in leading and presenting educational content to interdisciplinary community teams.

Bilingual in English/Spanish is a plus

Ability to work effectively in a team environment.

Knowledge of social work principles and practices, including case management and evidence-based short term therapy models

Skilled in advocating and connecting with internal and external resources to meet the patients’ needs

Supervisory Responsibility: This position will not have supervisory responsibility.

Travel requirements: Travel may be required up to 50% locally to deliver patient care.

Work Conditions: Ability to lift 20lbs. Ability to stand for extended periods. Ability to drive to patient locations (i.e., home, hospital, SNF, etc). Fine motor skills/Visual acuity.
