Licensed Practical Nurse (LPN) – Per Diem/Contract

Position Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care across a variety of healthcare settings, including hospitals, long-term care facilities, rehabilitation centers, and clinics. The LPN will be responsible for supporting patient care plans, administering medications, monitoring patient conditions, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. This is a per diem/contract position offering flexible scheduling opportunities.

Key Responsibilities:

Provide direct nursing care to patients under the supervision of a Registered Nurse (RN) or physician in accordance with state scope of practice regulations.

Administer medications, injections, and treatments as prescribed.

Monitor and document patients’ vital signs, symptoms, and overall condition, reporting any changes to the supervising clinician.

Assist with patient assessments and contribute to care planning.

Perform routine nursing procedures such as wound care, catheter care, and respiratory support.

Support patients with activities of daily living (ADLs) and ensure comfort and safety.

Accurately maintain medical records and documentation in compliance with facility and regulatory standards.

Collaborate with healthcare team members to ensure coordinated and effective care delivery.

Provide patient and family education regarding care plans, treatments, and health maintenance.

Adhere to infection control protocols and safety standards at all times.

Respond to medical emergencies and assist in crisis situations as needed.

Qualifications:

Current and valid LPN license in the state of practice (required).

Current CPR/BLS certification (required).

Minimum of 1 year of clinical nursing experience in a healthcare setting (hospital, long-term care, rehab, etc.).

Strong knowledge of nursing practices, medication administration, and patient care protocols.

Proficiency in electronic medical records (EMR) systems.

Excellent communication, organizational, and critical thinking skills.

Ability to work independently and as part of a multidisciplinary team.

Flexibility to work various shifts, including days, evenings, nights, weekends, and holidays as needed.Company Description
