Admissions Nurse

Title: Admissions Nurse

Location: Northeastern United States Region

Description:

The Admissions Nurse is responsible for evaluating and admitting patients referred for care services, while providing education to patients, caregivers, and families about available hospice services. This role involves assessing patients for eligibility, coordinating with physicians and interdisciplinary teams, and developing an initial plan of care. The Admissions Nurse also maintains communication with community partners, hospitals, and referral sources, and supports outreach efforts to build strong relationships within the healthcare community. Additionally, the role includes visiting hospitalized patients and ensuring continuity of care.

Required Skills:

Graduation from an accredited school of nursing; Bachelor of Science in Nursing preferred.

Active nursing license to practice in the designated region.

Minimum of two (2) years of experience in an acute care setting.

Working knowledge of community health services and entitlement programs.

Strong collaboration skills and ability to build relationships with healthcare partners.

Proficiency in documenting patient assessments and care plans in electronic medical records in a timely manner.

Ability to work within interdisciplinary teams and provide education to patients and caregivers.

Desired Skills:

At least one (1) year of hospice or palliative care experience preferred.

Knowledge of hospice admission protocols and care coordination.

Understanding of medical equipment and pharmaceutical needs related to patient care.

Ability to participate in on-call rotation and manage admissions assignments efficiently.

Compensation:

Base Salary Range: $109,000 - $140,000 per year
