Description

POSITION SUMMARY:

Responsible for delivering fitness programs and services to promote physical health and well-being in the community by creating culturally relevant innovative program designs, conducting individual and group exercise sessions, developing fitness plans, and educating participants on health and wellness topics.

ESSENTIAL FUNCTIONS:

Participates in department, division, or program events, training, or workshops;

Initiate and develop new fitness or wellness programs tailored to the community’s needs;

Creates intervention plan(s) including goals, objectives, and timelines for long-term program development;

Collaborate with management on proposals to secure funding for new or expanded programs;

Utilizes evidence-based research to design innovative fitness solutions;

Coordinates and leads basic group exercise activities (classes, boot camps, walks, hikes etc.) for youths, adults, and elderly;

Educate the community regarding physical activity guidelines and proper exercise techniques such as anatomical cues, muscle group identification, and movement mechanics to improve skills, equipment use;

Teach behavior modification techniques related to stress management, weight control, behavioral health and wellness, and other health topics;

Participates in various outreach efforts, such as health fairs, to promote exercise engagement and participant recruitment;

Assist with the development of educational materials such as brochures, flyers, and social media posts to inform the community about fitness opportunities.

Support participant engagement and relationship-building.

Implements and tracks exercise referrals from healthcare providers including patient fitness assessments, personalized plans, outcomes, and follow-ups in appropriate systems;

Documents patient education and progress in appropriate systems i.e. IHS Registration Patient Management System (RPMS), IHS Electronic Health Record (EHR), Southwest Indigenous Data (SID) Portal, or other);

Collects and reports data for all physical activities using appropriate databases, software, or systems;

Collaborate with management on program and/or grant required evaluation of services, reports and/or plans necessary modifications and improvements;

Serves as a subject matter expert to support interdisciplinary teams by maintaining best practices and safety standards in the prevention and management of chronic disease;

Participates in Pueblo Emergency Management Plan.

Perform other position related duties as required and assigned.

Qualifications

MINIMUM REQUIREMENTS:

Bachelor's Degree in Exercise Science, Kinesiology, or equivalent preferred and/or minimum of three (3) years of experience developing and implementing exercise interventions in a community health/wellness or clinical setting, chronic disease or preventive program; Must have a valid driver’s license and be insurable; Must be able to successfully pass a post-offer drug screen and criminal history background check.

KNOWLEDGE, SKILLS, AND ABILITIES:

Knowledge of principles and methods for exercise design, education, and instruction in individuals and groups; Knowledge of gait and body composition analysis; Knowledge of the measurement of exercise training effects; Knowledge of resting and exercise blood pressure and heart rate assessments; Knowledge of nutritional concepts; Knowledge of behavior modification and fitness for special populations groups; Knowledge of human anatomy and physiology; Knowledge of basic medical terminology concepts; Knowledge of medical history and movement assessments; Knowledgeable on safety procedures and operations of health fitness equipment; Knowledge of HIPPA requirement; Excellent interpersonal and communication skills, both orally and written; Effective time management; Ability to analyze and problem solve; Ability to work as part of a multi-disciplinary team; Ability to use technology including use of laptops, smartphones, internet/online application systems; Flexibility to meet community needs; Must be energetic with outgoing personality; Committed to providing excellent customer service; Must be culturally sensitive to minority population especially Native Americans; Position requires poise, tact and diplomacy; Ability to occasionally travel for trainings.

PHYSICAL REQUIREMENTS:

While performing the essential functions of this position, the employee is frequently required to sit, talk, lift, bend, and hear. The employee is occasionally required to reach with hands and arms. The employee is occasionally required to lift 100+ pounds. The vision requirements include close vision, peripheral vision and ability to adjust focus.
