Corporate Care Management, Inc. (CCM) is seeking a compassionate and dedicated Care Coordinator to join our team in the workers compensation field. Duties include assisting injured workers in accessing needed medical services and coordinating their care in effort to avoid unnecessary delays in treatment and potentiate return to work in good health. The ideal candidate is a motivated and driven individual with excellent customer service and verbal and written communication skills who is proactive, self-disciplined and able to work effectively both independently and as part of a team.
Key responsibilities of a Workers' Compensation Nurse Care Coordinator typically include:
Coordination of Care:
Coordinating with healthcare providers to ensure the worker receives proper treatment and follow-up care.
Facilitating Communication:
Serving as a liaison between the injured worker, the employer, healthcare providers, and insurance companies.
Ensuring that all parties are informed of the worker’s progress and treatment plan.
Monitoring Progress and Recovery:
Tracking the worker's recovery process, including follow-up appointments and treatments.
Assisting claimants with following the treating providers care plans to ensure the worker’s optimal recovery and safe return to work.
Ensuring Compliance:
Assisting adjuster to ensure that medical treatment complies with workers’ compensation guidelines and state regulations.
Working in collaboration with the employer to ensure line with the employer’s return-to-work policies and the insurance carrier’s requirements.
Managing the Workers’ Compensation Claim:
Helping manage and minimize the costs associated with the workers’ compensation claim while ensuring the worker receives appropriate care.
Advocacy:
Advocating for the injured worker’s best interests while ensuring a smooth recovery process.
Requirements:
· An active, unrestricted NYS LPN license with a minimum of 2-3 years of experience
· Excellent communication and interpersonal skills for interacting with patients, healthcare providers
and insurance adjusters.
· Strong organizational and critical thinking skills with the ability to manage multiple cases simultaneously.
· Experience with workers’ compensation is a plus but not required. We are open to candidates with diverse backgrounds and will provide training as needed.
· Availability Monday through Friday, 8am to 4pm EST
