Looking for a Part-time or Full-Time Social Worker to join our team!

The Kidney Care Center, located in Hobbs, NM, is a 16-chair hemodialysis center. We also offer Peritoneal Dialysis for patients.

We are seeking a highly motivated and dedicated social worker to join our dialysis team in New Mexico. The ideal candidate will have experience in providing social work services to patients with end-stage renal disease and will possess excellent communication and interpersonal skills.

Job Description

How you will change lives

As a social worker at The Kidney Care Center, you will be an integral part of a cross-functional

team, working with healthcare providers to facilitate quality care for patients living with kidney disease.

What you will be doing

Provide emotional support and counseling to patients and their families

Assess patient and family psychosocial needs and develop care plans

Coordinate patient care with other healthcare professionals

Advocate for patients and their families in accessing community resources

Participate in interdisciplinary team meetings and patient care conferences

Maintain accurate and up-to-date patient records

Qualifications/Skills:

Master's degree in Social Work from an accredited institution

Current state licensure as a Social Worker required

Experience in providing social work services to patients with end-stage renal disease preferred

Knowledge of Medicare and Medicaid regulations and policies related to dialysis

Strong communication and interpersonal skills

Ability to work independently and as part of a team

Excellent organizational skills

Bilingual- Spanish speaking preferred
