Conduct up to hour-long assessments covering medical history, social factors, and health status in clients' homes, SNFs, or community settings.
Collaborate with physicians, social workers, and caregivers to develop care plans and provide follow-up.
Qualifications include a Master s degree or equivalent, current state licensure, certification, and prescriptive authority.
Flexible schedule with day, evening, night, and weekend hours; travel required.
No benefits, but mileage reimbursement and potential monthly earnings from $2,000-$10,000 based on completed assessments.
