Description:Position Summary:

On behalf of Celebrity Home Health, Inc., this position is responsible for overseeing State clinical compliance and ensuring the continued licensure of the Home Health Agency (HHA) in good standing within the state.

Key Responsibilities:

Ensure regular attendance and punctuality; demonstrate the ability to work collaboratively across all levels of the organization.

Implement quality improvement initiatives to enhance patient care.

Oversee state licensure compliance and adherence to all relevant governmental regulations.

Establish and maintain a high standard of professional and ethical practices for all home care staff.

Delegate tasks and responsibilities effectively to the appropriate personnel.

Ensure State supervisory personnel are properly hired, trained, and meet the highest standards.

Collaborate with the Quality Assurance Program RN to ensure patient records are completed accurately, meeting regulatory, corporate, and payer requirements.

Lead or co-lead the State Quality Assurance Committee.

Contribute to initiatives such as Infection Control, Performance Improvement, and Customer Satisfaction.

Serve as the clinical point of contact for all regulatory surveys and preparation, offering guidance to the State Administrator.

Provide clinical input for executive decisions and serve as a resource for the State Administrator.

Participate in public relations and community outreach to promote Celebrity Home Health’s role in delivering quality in-home care services within the region.

Ensure complete documentation in cases of employee separation, outlining reasons for each.

Adhere to corporate policies regarding on-the-job injuries and workers' compensation claims.

Provide direct patient care as required by state regulations, including initial evaluations and regular re-assessments.

Assist with implementing Plan of Care changes to improve client outcomes.

Promote and maintain client health through education and appropriate rehabilitative measures.

Coordinate services with other healthcare providers as needed.

Conduct chart reviews, audits, and monitoring of patient records to ensure compliance with state regulations.

Ensure that staff competencies are assessed at hire and annually as required.

Maintain clinical compliance and utilization metrics of 95% or above.

Perform additional duties as needed or as stipulated by state regulations.Clearances:

- Bilingual Spanish - English Preferred

Associate’s Degree in a related field

Registered Nurse (RN) license to practice professional nursing in all states where patient care is provided

Basic Life Support (BLS) certification

Current state licensure and current liability/malpractice insurance (if applicable)Preferred Qualifications:

5+ years of experience as a Registered Nurse

Nurse Management/Leadership experience

Previous experience in home health care

Experience with Department of Labor (DOL) and EEOICPA programsLicensure/Certification:

Valid Driver’s License

Current CPR Certification

Valid RN license in all states where patient care is provided
