Job Description
Responsible for recruitment, information and referral to resources for ex-offenders. Maintaining contact with clients in the community in an effort to offer support and guidance and linking clients with needed services to promote employment, treatment, and housing.
Scope of Duties:
Recruit clients to participate in program including visiting the county jail to interview prospective clients.
Complete intake interviews with clients and secure program participation agreements.
Assist clients apply for social entitlements such as GA, TANF, etc. when appropriate.
Link homeless clients to housing.
Monitor and document client progress.
Assess client needs and work with client to develop goals.
Update service plan with client input.
Provide motivational support to clients in program and maintain support throughout participation.
Schedule and facilitate meetings with clients as required.
Identify and/or develop resources and link clients to needed services.
Facilitate scheduled workshops/programs/groups addressing client's needs.
Develop appropriate discharge plan on all assigned clients.
Participate in required internal/external meetings.
Complete all required reports, data collection and other documentation.
Interact with all clients, staff, and outside contacts with respect and courtesy.
Maintain client and staff confidentiality in compliance with administrative policies and procedures.
Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
Other duties as assigned.
