Job Description
Duties and Responsibilities:
Excellent customer service skills and ability to deal tactfully with clients, other
interdisciplinary team members
ー As a brand ambassador the LVN treats client and team members with courtesy and respect.
ー Reviews the care plans arranged by the case manager (Care Designer) prior to beginning a visit, completing records accurately and carrying out all assignments as requested
ー Responds to Care Services Center (CSC) Team regarding scheduling inquiries and updates within the agreed upon timeline
ー Engages in open communication with your Care Designer when opportunities to
elevate your client’s experience and care are identified
ー Delivers comprehensive professional nursing care consistent with the care plan,
nursing assessments and client needs.
ー Perform all skills, treatments and procedures competently in accordance to the
provincial scope of LVN and adheres to the policies, procedures and protocols of Nurse Next Door.
ー Assess and provide client and family/caregiver education and information pertinent to diagnosis.
ー Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth.
Requirements and Experience:
ー Current Certificate of Competence and in good standing with the provincial College of Nurses
ー Graduation from a recognized program and one year of recent related experience.
ー Functions within the limits of own experience and knowledge, and practices safely and competently within the job description.
ー 1 year of nursing experience preferred.
_Community, home health or medical/surgical experience preferred.
ー Criminal Record Check with a Vulnerable Sector Search (updated at the nurses cost every 5 years for continued employment)
ー Negative TB test or chest x-ray (updated at the nurses cost every 5 years for continued employment)
ー Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth.
