We are seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA-C) to join our team at a Rehabilitation Center in Nashua NH. This role offers a unique 7 days on / 7 days off schedule, providing an excellent work-life balance. The ideal candidate will have 1-2 years of experience as an Advanced Practice Provider (APP) and a passion for providing high-quality rehabilitative and post-acute care.

Key Responsibilities:

Conduct comprehensive patient assessments, develop treatment plans, and provide ongoing medical management.

Collaborate with an interdisciplinary team, including physicians, therapists, and nursing staff, to ensure optimal patient outcomes.

Monitor patient progress, adjust care plans as needed, and provide patient and family education.

Order and interpret diagnostic tests, prescribe medications, and perform minor procedures within scope of practice.

Ensure compliance with all regulatory and facility guidelines.

Qualifications & Requirements:

1-2 years of experience as a Nurse Practitioner or Physician Assistant (required).

Current NP or PA license in NH.

Experience in rehabilitation, post-acute, or skilled nursing facility (SNF) settings is preferred.

Strong clinical skills, critical thinking, and ability to work independently.

Excellent communication and collaboration skills.
