POSITION SUMMARY: A QMHP is responsible for the assessment, diagnosis, and treatment of individuals and families affected by mental illness and emotional issues. Must be able to coordinate the assessment, counseling, and referral of clients as well as work within a multidisciplinary team to treatment plan for the best client outcomes.

QUALIFICATIONS:

Education: QMHPs shall meet the following minimum qualifications: (a) Bachelor's degree in nursing and licensed by the State of Oregon. Nurses are accountable to abide by the Oregon Nurse Practice Act to determine if job descriptions are compliant with the competencies listed above; (b) Bachelor's degree in occupational therapy and licensed by the State of Oregon; (c) Graduate degree in psychology; (d) Graduate degree in social work; (e) Graduate degree in recreational, art, or music therapy: (f) Graduate degree in a behavioral science field; (g) A qualified Mental Health Intern, as defined in .

Licensure/Certification: Licensed Professional Counselor (LPC) by the State of Oregon Board of Licensed Professional Counselors and Therapist, or Licensed Clinical Social Worker (LCSW) in the State of Oregon, or another license as listed above. Must pass a criminal history check as required by ORS 443.004. Valid Oregon Driver's license with a clean driving record for the past three years. Have or obtain CPR/BLS certification.

Experience: At least two (2) years' experience in mental health programs, required. Experience in differential diagnosing, treatment plan design and implementation with children, adults, and families. Experience and understanding of rural service delivery and community partnership preferred. Understanding of best practices in the delivery of community-based mental health services within both public and private managed care reimbursement systems. Experience working with the courts, law enforcement, schools, and a variety of community partners. EHR systems experience required. Microsoft office suite experience required.

Job Knowledge: Must be able to demonstrate the ability to conduct a mental health assessment to include identifying precipitating events, health and safety risks to self or others; gather histories of mental and physical health, substance use, past mental health services, and criminal justice contacts; assess family, cultural, social, and work relationships; conduct a mental status examination; complete a DSM-V-TR diagnosis; develop a safety plan; write and supervise the implementation of a person-centered treatment plan; and provide individual, family, or group therapy within the scope of training.

Expected Hours of Work: Hours of typical operations are M - F, 8:00 am to 5:00 pm. Must be able to work a flexible schedule, as needed. On-call rotation required.

Skills: Must be able to work with people in a respectful and professional manner. Must have excellent communication skills. Must have effective time management and organization skills.

Equipment (PPE): (PPE) required to perform the job safely.

Supervisory Responsibility: This position has no direct supervisory responsibility of staff, however, review of documentation and services performed by Qualified Mental Health Associates (QMHA) may be necessary.

Travel: Travel (using a company vehicle) occasionally required for this position.

ESSENTIAL FUNCTIONS

Assess clients and diagnose mental health concerns.

Create treatment plans for clients that are person-centered and based on the needs of the individual.

Provide counseling services within scope of practice.

Refer clients to other healthcare professionals when necessary.

Monitor client progress and modify treatment plans as needed.

Work as a member of a multidisciplinary team to effectively coordinate services with case managers, peer support specialists, alcohol & drug counselors, and other community resources.

Maintain thorough records of client progress. Monitor client progress and adherence to treatment plan. Maintain client files and case documentation in accordance with related policies.

Establish and maintain working relationships with community resources and agencies. Utilize and make referrals to assist client in plans for education, vocational training, employment, housing, etc.

Provide prevention-focused activities to individuals and groups.

Maintain a caseload of 36 - 60 clients depending on community need.

Maintain a 60% productivity rate.

Participate in research-based activities as indicated.

Share in after hours on-call rotation. Intervene during client's crisis situations.

Maintain all safety rules and procedures for work areas.

Maintain client confidentiality.

Complete continuing education courses to maintain licensure within the scope of practice.

Provide telehealth services in accordance with practice standards.

HIPAA Compliance: The Health Insurance Portability and Accountability Act of 1996 (HIPPA) is a federal law which, in part, protects the privacy of individually identifiable patient information, provides for the electronic and physical security of health and patient medical information. Patient information of any kind must be kept confidential. Confidentiality and compliance with HIPAA, Federal Confidentiality Regulations (42 CFR Part 2), and State Confidentiality Regulations as specified in ORS 179.505 and 192.518 through 192.530 must be followed.

WORK ENVIRONMENT/PHSYCIAL DEMANDS/LIMITATIONS/HAZARDS (ADA):

Regularly required to use Microsoft Office Suite and other digital technology that require fine manipulation, grasping, typing, and reaching mobility.

Manual dexterity and coordination are required over 50% of the work period while operating keyboard and digital equipment.

Frequently or occasionally required to sit, squat when adjusting equipment or retrieving items, stand, walk, bend, kneel, stoop, reach with hands and arms, and manipulate objects.

Duties involve moving materials weighing up to 10 pounds on a regular basis such as books, office equipment, etc. and may infrequently require moving materials weighing up to 25 pounds.

Specific vision abilities required by this position include close vision, peripheral vision, distance vision, and the ability to adjust focus.
