MDS Coordinator RN or LPN

Job Description

Gallatin Manor Nursing & Rehabilitation is seeking an MDS Coordinator RN or LPN to fill a full-time, salaried position in our skilled nursing facility.

The MDS Coordinator RN or LPN duties will be making sure that all paperwork is completed, and the facility is receiving the maximum reimbursements entitled. You will collect data and gather information from staff and resident to create a detailed, accurate and comprehensive clinical assessment.

Don't forget...

An active state RN or LPN as applicable license is required.

Understanding of the MDS process: coordination, scheduling and completion.

SNF/long-term care experience.

This is a direct hire position; we are not an agency.

Salary

LPN: $75,000 per year

RN: $85,000 per year

*MDS Coordinator, RN or LPN salary increases are tied to census*
