*Position Overview: Physical Therapist (PT)*

As a Physical Therapist at Pinnacle Home Care, you’ll play a vital role in delivering high-quality care to patients in the comfort of their homes. You’ll work under the direction of a physician and nursing supervisor, ensuring care meets all regulatory and clinical practice standards.

*Key Responsibilities*

* Perform initial and ongoing patient assessments, including OASIS documentation.

* Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.

* Collaborate with the interdisciplinary care team to optimize patient outcomes.

* Provide education and support to patients and families on therapy programs.

* Supervise Physical Therapist Assistants every 30 days.

* Communicate patient progress and changes with physicians and care teams.

* Document all visits and progress notes on the same day of service.

*Qualifications*

* Graduation from an accredited Physical Therapy program.

* Active PT license in Florida.

* Current BLS certification.

* Valid driver’s license and reliable, insured vehicle.

* At least one year of PT experience (home health experience preferred).

* Proficiency with electronic medical records is a plus.

* Physical capability to perform required tasks (standing, bending, lifting).

* Strong self-management, communication, and judgment skills.
