Lifespark - Float MDS Coordinator, RN

Seeking a dedicated MDS Coordinator to join a skilled nursing community, floating between Forest Lake and Woodbury locations.

Full-time position with a wage of $44-$47/hour based on experience, plus a $2,500 sign-on bonus.

Responsibilities include overseeing MDS documentation, ensuring accuracy, compliance, and timely completion, educating staff, and coordinating assessments with the interdisciplinary team.

Qualifications: Valid RN license and at least one year of long-term care experience preferred.
