What You’ll Do:
Our Case Manager make a difference in the lives of our patients. Our dedicated professionals work tirelessly to create an environment that fosters healing, growth, and long-term wellness. The Case Manager provides quality case management, psychotherapeutic services, and discharge planning to all patient and their families, serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and their families.

Aurora Charter Oak Hospital has been delivering Behavioral Health Treatment Services for over 83 years. We are located on 10 acres of beautifully landscaped grounds in Covina, California. Covina is about 22 miles east of downtown Los Angeles, in the San Gabriel Valley, close to 10, 60 and 210 FWY.

Requirements
Education & Experience:
· Bachelors Degree in Social Work, Psychology or Social Services
· Master’s Degree preferred
· 3 years behavioral health experience preferred
· Associate license such as ASW (Associate Clinical Social Worker), AMFT (Associate Marriage & Family Therapist), APCC (Associate Professional Clinical Counselor) preferred
· Current BLS healthcare provider card required upon hire
· Therapeutic Options™ training to be obtained prior to providing direct patient care without a preceptor
