ChildNet, Inc. is a private, not-for-profit community-based care (CBC) agency servicing Broward and Palm Beach County. We are contracted with the State of Florida, Department of Children and Families, to provide case management support to abused, abandoned, and neglected children. ChildNet is nationally accredited by the Council on Accreditation (COA) and employs over 450 staff at its location in Ft Lauderdale and West Palm Beach, Florida.

Job Summary: This position serves as a liaison between client services, CBCIH and Sunshine Health to coordinate and schedule the medical screenings (EPSDT) and dental appointments for children in out-of-home care, and in cooperation with the Child Advocates (CAs) and Dependency Case Managers (DCMs).

Collaborates with Child Advocates and Dependency Case Managers to provide referrals and follow-up appointments with physicians, specialists and dentists for children based on recommendations made during medical screenings.

Assist foster parents, relatives or non-relative caregivers with medical care and dental visits.

Coordinate the transportation for children to medical and dental service providers.

Adjust medical plans to provide the highest level of care for children. Follow up with prescription orders and verify the orders are received and picked up by the caretaker.

Perform initial Health Risk Assessments for children in out of home placements. Identify network PCP for newly enrolled and coordinate ongoing PCP changes with CBCIH. Assist in the development of care plans. Update FSFN with medical and dental information.

Participate in utilization management integrated care team and multidisciplinary care team meetings.

Advise Child Advocates and Dependency Case Managers on complex case management issues regarding medical insurance and coverage.

Maintain a high level of confidentiality when dealing with sensitive client information.

Performs other duties as assigned.

Skills/ Requirements:

Years of Experience:

Two (2) years of experience in a Health Coordination or related field

Bilingual Spanish Preferred

Education/Licenses/Certifications:

High School Diploma or GED required.

Abilities Required:

Ability to make complex decisions and balance the needs of stakeholders involved.

Knowledge of Microsoft suite programs.

Ability to organize and prioritize tasks.
