Summary, Job Description, and Qualifications
Job Title: Supervisor - OhioRISE CME Care Coordination
Reports To: OhioRISE CME Program Director
Direct Reports: OhioRISE Care Coordinators I and II
Summary:
Applying the principles of Systems of Care, the Supervisor of OhioRise Care Coordination is responsible for developing and overseeing a team of Care Coordinators who ensure and cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for Tier II Moderate clients and Tier III Intensive clients in alignment with The Village Network’s Mission, Vision and Core Values.
Essential Tasks, Duties, and Responsibilities:
Participates in the recruitment, interviewing and onboarding of new Care Coordinators; Assists in the delivery of orientation training and supports learning during staff probationary period in alignment with the standards for OhioRISE.
Supervises care coordinator teams consisting of 8 direct reports; Provides one-on-one progress and communication meetings with individual staff members at least one time per month; Facilitates team meetings, at last one time per month; provides both formal and informal training for direct reports that is individualized, specialized and ongoing to support professional development in their role.
Ensures achievement of required metrics in the delivery of care coordination including but not limited to: face to face contacts and the timely completion of required documents for each OhioRISE member.
Provides monthly utilization review for the team; Review monthly claims and metric reports to promote team strategies to meet monthly and annual goals.
Conducts performance evaluations and develops action plans for assigned staff for continued professional development. Addresses performance in alignment with TVN standard practices of recognition and progressive discipline. Develops, implements and evaluates corrective action plans as needed.
Reviews and approves required forms including but not limited to CANS, care plans, safety plans, assessments, activity trackers to ensure ongoing quality with documentation and consistent delivery of care.
Ensures adequate staff coverage and crisis coverage for clients in cases of staff absences.
Ensures development of an appropriate aftercare plan regarding ongoing client needs; Oversees the transition of care process; Assures each direct report achieves caseload capacity.
Participates in meetings facilitated by ODM, Aetna, community partners, as directed; Upon request, attends all scheduled meetings including but not limited to: clinical escalation, fidelity reviews, MSY, PRTF, and child and family team meetings.
Participate in required fidelity reviews as coordinated by Case Western’s Child and Adolescent Behavioral Health Center of Excellence. Provide data as requested for the completion of those reviews
Complete written work that is legible, accurate, uses professional language, and meets established timelines.
Other duties as assigned.
Knowledge, Skills, and Abilities:
Bachelor’s degree with 5 years’ experience, or Master’s degree or higher with 1 year of experience, in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
May be licensed or an unlicensed practitioner; Licensure preferred; If unlicensed with the bachelor’s degree, the experience requirement must include a minimum of 2 years of specific OhioRISE Care Coordination or high fidelity wrap experience.
Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
Must complete the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM.
Must successfully complete skill and competency-based training to supervise delivery of Intensive and Moderate Care Coordination (ICC/MCC).
Be culturally competent or responsive with training and experience necessary to manage complex cases.
Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child-serving systems (e.g., child welfare, juvenile justice, education).
Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).
Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
Willingness to travel for various reasons, mainly during the day but occasionally overnight. Ability to visit clients’ homes (may or may not be handicapped accessible).
Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
Maintain a HIPPA compliant home office
Physical Demands:
Occasionally move about inside the office to access file cabinets, office machinery, etc.
Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.
Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
Must be able to lift 20 pounds independently
