Join a highly respected non-profit organization dedicated to providing impactful healthcare services to underserved communities. This role offers an incredible opportunity for a compassionate Primary Care Provider (APP) to make a meaningful difference in the lives of youth at a transitional housing site in the Bronx.

Position Highlights:

Schedule: Monday-Friday

Hours: 9-5, or 8-4, some flexibility!

Setting: Transitional Housing

Position Type: Full-time, permanent

Key Responsibilities:

Provides medical care to pediatric, adolescent and adult patients, takes relevant patient history, and identifies patient needs and makes diagnoses

Observes, implements, and promotes the federal regulations as defined by HIPAA and Policies and Procedures

Assists the Chief Medical Officer in the development and implementation of medical protocols and medical forms

Works collaboratively with physicians and specialists for consultation and referral purposes

Qualifications:

2+ year of relevant experience as an Advanced Practice Provider

Active NYS PA or FNP license.

Board/DEA certified with access to Medicaid

Passion for working with underserved or at-risk populations.
