Job Overview:
We are seeking a dedicated and skilled Registered Nurse with at least 1 year of Home Health experience and a minimum of 3 months of OASIS documentation experience. This is a travel position with potential floating assignments to neighboring islands (all related expenses covered). The ideal candidate will be experienced in managing a wide range of clinical responsibilities in the home health setting and committed to delivering high-quality, patient-centered care.
Key Responsibilities:
Provide skilled nursing care in patients' homes in accordance with physician orders and care plans
Perform comprehensive assessments, including Start of Care, Resumption of Care, Recertification, and Discharge assessments
Manage and monitor a wide range of patient conditions and clinical needs, including:
Wound care, Wound V.A.C. management, Pressure ulcer prevention/staging/management
Pain assessment and management (including hospice/palliative care)
Blood glucose monitoring and glucometer use
G-tube, NG tube, Colostomy/Ileostomy, and Suprapubic catheter care
IV management, including peripheral/central lines and port access
Post-operative care (e.g., amputations, joint replacements, AV fistula/shunt)
Administration of blood products
Nutritional assessments and medication management
Coordinate care services with other healthcare professionals
Ensure accurate clinical documentation and maintain compliance with infection control and safety standards
Maintain productivity expectation of 25 units per week
Requirements:
Active Hawaii RN License (pending license applications considered; note: no temporary license option for travelers)
Minimum 1 year of Home Health nursing experience
At least 3 months of recent OASIS documentation experience
BLS Certification (AHA required)
Strong clinical assessment, case management, and patient education skills
Proficiency in handling medical equipment and wound care technologies
Excellent communication, time management, and organizational skills
