Job Description Summary:
Supports mobile crisis response to facilitate crisis intervention with children and families and assists with the management of psychiatric and behavioral health concerns occurring within various settings. Provides skill building, psychoeducation, support, linkage, and referral interventions that promote resilience, independence, and attention to the stabilization of the youth and family unit following a crisis.

Essential Functions:
Supports licensed provider in culturally appropriate crisis-oriented triage, assessment, and other appropriate mental health screenings via mobile response to appropriate children and families.
Facilitates appropriate brief crisis interventions, care coordination, safety planning, or other therapeutic interventions to assigned patients, at direction of licensed clinician. Provides face-to-face and phone follow up and support to youth and family following a mobile response. Completes required charting and/or clinical documentation in accordance with NCH and applicable program standards.
Collaborates with youth and family to implement a treatment plan at direction of licensed clinician. Assists with care coordination and collaboration with current behavioral health providers, natural supports, and other systems involved with youth and family. Completes physical outreaches as clinically indicated.
Provides short-term case management support to assist with monitoring of symptoms, stressors, level of risk, safety planning, and de-escalation during stabilization phase. Utilizes MRSS work queue to coordinate case management needs for patients.
Assists with identifying and building skills, social supports, accessing resources, and services to assist with stabilizing the crisis and preventing future crises from occurring. Works collaboratively with other members of the MRSS team and assists with a warm handoff to long-term behavioral health providers and supports, including completion of a transition plan.
Maintains currency in required training and familiarity with the research and literature in areas of expertise.
Education Requirement:
Bachelor's Degree in relevant field, required.
Licensure Requirement:
QBHS certification to be obtained upon hire.
Certifications:
Completion of NCH endorsed Crisis Management Training upon hire.
Skills:
Demonstrated skills in providing behavioral health interventions and case management to children and adolescents and their families.
Demonstrated ability to exercise a degree of independence, initiative, judgment, and decision-making to prioritize workload and achieve program objectives.
Ability to work both independently and collaboratively as a team member to ensure consistent quality of services that support a high level of patient and provider satisfaction.
Demonstrated communication and interpersonal skills required to establish and maintain productive and collaborative team and multi-disciplinary relationships.
Demonstrates sensitivity, empathy, objectivity, professionalism, effective engagement and customer service skills, and sound judgement in the course of all work duties.
Embraces principles of family centered care in which needs, and perspectives of patients and families are acknowledged and honored.
Experience:
One year of providing crisis-based mental health services with children and adolescents, preferred.
