Clinical RN Opportunity in New Brunswick, NJ. 1. Provides medical case management for patient population includes HIV exposed infants, HIV infected children and adults and pregnant women. 2. Assesses the health status of patients assigned. 3. Provides direct patient care for patients including vital signs, immunizations, STI treatment if necessary. 4. Reviews medications and laboratory/imaging test results with physician. Enters lab orders and medication refills. 5. Maintains communication with patient about disease status, medications, medication adherence and disease education. 6. Provides phone/text/email and/or written correspondence for patient appointments and missed appointments. 7. Schedules all medical appointments and arranges transportation for patients if necessary. 8. Prepares clinic visit prep form for outpatient visits. 9. Documents all aspects of patient care. 11. Provides referral and linkage information for patient appointment including: date, time, location of specialty visits. 12. Prepares individual patient correspondence—letters to other physicians etc. 13 . Prepares medical information for patient paperwork i.e. SSI, Disability and ADDP. 14. Obtains patient medical records as needed. 15. Provides medical case management for HIV + pregnant women in collaboration with prenatal providers. 16. Provides coverage for other staff as needed. Manage assigned caseload of patients. a. Actively participate quality improvement initiatives b. Takes the initiative to understand the needs of all patients c. Identifies barriers that inhibit patient care and works to remove them.. d. Works towards achieving grant quality measures 18. PATIENT EDUCATION: a) Provides risk-reduction and HIV disease education for infected and affected patients. b) Provides medication education and medication adherence strategies for patient population. · c) Provides disease education and disease management strategies for patients and families. 19. DATA AND REPORTS REQUIREMENTS: a) Provides data requirements for RWJAP including: daily patient encounter forms, new patient data form, monthly patient calendar and others as needed. b) Provides statistical and narrative report data for quarterly/annual reports as requested. c) Prepares monthly perinatal updates for database d) Maintains patient lab data and updates on restricted drive REQUIREMENTS: Registered Nurse with at least three years nursing experience. Basic computer skills: proficient with EMR, entering data on spreadsheets, Microsoft Word, and email. 35 hours 5 days 7.5 or 4 days 8.5 must be in office Friday M-F
