POSITION SUMMARY: Under the supervision of the TelePrEP/Access to Healthcare Program Manager, the Peer Navigator, supports clients accessing PEP and PrEP services.
This role is essential in increasing awareness and access to HIV prevention strategies through telehealth and in-person interactions.
The Peer Navigator will assist with outreach, appointment reminders, insurance navigation, peer counseling, and PrEP education.
This position is fully funded by the PrEP Telehealth program, with no shared responsibilities.
All time and effort under this role are allocated exclusively to the PrEP Telehealth program.
DUTIES AND RESPONSIBILITIES: * Assist clients in accessing PEP and PrEP services through telehealth and in-person visits.
* Provide guidance on virtual healthcare services and assist with telehealth appointment completion.
* Engages with community members in laundromats, beauty salons, other local businesses churches, parks, and other public spaces to educate about PrEP and telehealth services.
Uses technological equipment to facilitate real-time telehealth PrEP visits, expanding access to care in non-traditional settings.
* Support health insurance navigation, including enrollment in health plans and medication assistance programs.
* Assist with appointment reminders and follow-ups for missed visits.
* Maintain confidentiality and privacy while providing resources and referrals for continuous PEP/PrEP access.
* Collaborate with clinic staff to coordinate telehealth and in-person services.
* Participate in training sessions and support programs for peer educators.
* Support basic data collection for CHN's Electronic Medical Records (EMR) and AIDS Institute Reporting.
* Perform other related duties as assigned.
EDUCATION & EXPERIENCE: * High School diploma or satisfactory equivalent required KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: * Strong knowledge of PrEP and HIV prevention strategies.
* Excellent communication skills, both verbal and written.
* Basic proficiency in Microsoft Word and Excel.
* Ability to provide peer support and patient navigation with empathy and professionalism.
* Respect for client confidentiality and autonomy in healthcare decisions.
* Experience in community outreach and engagement, including social media and digital communication a plus.
* Bilingual Spanish proficiency preferred.
