Schedule: Full Time 40 hours/week 8am-4:30pm, Monday-Friday Department: Thome PACE Jackson, MI What You'll Do in This Role: The Homecare Scheduling Assistant will be responsible for scheduling homecare services for PACE participants.
This role involves coordinating caregiver assignments, ensuring that services are provided in a timely and efficient manner, and maintaining accurate schedules for both caregivers and clients.
Thome PACE serves Jackson, Lenawee, and Hillsdale counties providing participants with a wide range of medical and supportive services in our Day Center located in Jackson, Michigan or in the comfort of their own homes.
Primary Responsibilities: * Maintains schedule for CENA's/PCA team members.
* Communicates with Participant Care Manager regarding CENA/PCA pick-ups/trade slips.
* Communicates with participants and/or families regarding homecare schedule or tasks changes.
* Maintains service notes related to services provided to participants.
* Works with Pest Control Agencies for infestation needs.
* Works with contracted agencies to secure in-home staffing as needed.
* Works with CNA staffing with communication of care needs, scheduling questions/concerns.
* Coordinate, monitor, and authorize care with GTI agency.
* Support Life Alert agency and coordination.
* May assist in participant homes when needed (if qualified). * Demonstrates active participation in QI processes such as chart review and care plans.
* Communicates participant changes with team members and participant/family member.
* Participates in care planning as needed.
* Attends staff meetings.
What You'll Need: * Required Qualifications: * High school diploma or GED - a strong foundation to build your career with us.
* Excellent written and verbal communication skills to effectively connect with our clients and team.
* Sharp problem-solving abilities with a knack for analytical thinking.
* A respectful and clear communicator, especially when working with clients, caregivers, and colleagues.
* CPR certification - ensuring you're prepared to respond when it matters most.
* Minimum of one (1) year of hands-on experience supporting frail or elderly individuals, bringing compassion and expertise to every interaction.
* Preferred Qualifications: * Certified Nursing Assistant (CNA) certification - an added advantage that enhances your care capabilities.
* Previous experience in scheduling within a home-based healthcare setting - helping us deliver seamless, personalized care to our clients.
