Case Manager

Full Time Days

Lutheran Social Services of Central Ohio is currently seeking a Case Manager for Faith Mission in Columbus Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the ADAMH Case Manager with Lutheran Social Services?

Works with clients on a regular and ongoing basis to develop and maintains an on-going plan of action creating individual goals by assessing client strengths and needs, client responsibilities, and action steps to help client in achieving their goals. This includes provides budgeting and other planning assistance toward goal attainment such as assisting clients in the development of job and life skills as needed, how to maintain their apartment, pay bills on time, apply for benefits, develop a budget and how to keep a job, etc.

Makes appropriate client referrals and linkages to community resources and support systems as needed and assists clients in securing stable housing and income. Acts as a liaison and link for clients with outside community resources.

Conducts intake interview to determine program eligibility from walk in and referrals. Maintains waiting list as appropriate and makes referrals to other organizations as appropriate. Completes required paperwork, communicating boundaries, rules, services and limitations of the program.

Conducts outreach efforts to external partners and organizations providing education about programs and services available to ensure maximum access and utilization by eligible participants. Solicit, collect and organize area donations from the community.

Stays up to date with community resources available to clients.

Assists clients as needed with transportation to move into housing, grocery shopping, etc, by accessing Lancaster Transit and other options when possible.

Establishes and maintains file, ensures timely, accurate, and complete case notes and documentation and statistics as required for the program including detailed documentation of meeting dates, referrals, case management hours, etc.

Keeps a close eye on direct client assistance to conform to budget. Keeps accurate mileage documentation.

Perform all other duties as requested.

Requirements for the ADAMH Case Manager with Lutheran Social Services:

Requires a Bachelor's degree in Social Work or related field.

2-4 years of case management and/or crisis intervention experience.

Experience with homeless or other special needs populations preferred.

Flexibility in managing schedule including weekends and evening hours.

Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required
