Job Description:
Position Overview:
Reporting to the Director of the Women and Children’s Team, the Nurse Practitioner (NP) role in the Sickle Cell Disease program is responsible for providing clinical care to support both paediatric and adult patients at SHN. The NP will work in both the paediatric and adult Sickle Cell Disease clinics with the interprofessional teams to provide seamless care for patients with Sickle Cell Disease. The NP will take a leadership role to develop, enhance, and initiate care pathways in the ED, inpatient, and clinic areas for this patient population with a direct follow-up with specialists and primary care providers. The ideal candidate will have a commitment to learning about inequities and social issues encountered by patients with Sickle Cell Disease and developing action plans and collaborating with community partners in the care plan.
Responsibilities:
Practice in accordance with the Regulated Health Professions Act, the Practice Standards set by the College of Nurses of Ontario and standards set out for the Nurse Practitioner
Perform patient assessments, medical history, physical, and psychosocial assessments
Develop and monitor care plans with patients/families, cross-department and the interprofessional team
Provide comprehensive assessments, interventions, monitoring, and follow up for acute and chronic illnesses related to Sickle Cell Disease
Actively participate in the Sickle Cell Disease Community of Practice and Sickle Cell Disease committee meetings
Develop linkages with external partners as appropriate to ensure continuity of care
Provide input and support in the development and implementation of Ontario Health’s Sickle Cell Disease Quality Standards
Provide follow-up support to patients in the ED, inpatient unit, and outpatient setting when appropriate
Liaise with community partners, specialists and primary care providers in the continuum of care for SHN patients
Assist in reducing emergency department re-admits and facilitate expedited transfers from the emergency departments
Provide staff education for capacity building in the ED, inpatient, and outpatient teams to enhance competency for care management
Provide leadership and promote excellence in patient-centred care through expert practice, education, consultation, research, and quality improvement
Document and utilize reports utilizing SHN Clinical Information System - EPIC
Utilize the protective equipment provided and identifies and reports hazards
Requirements:
Master of Nursing or equivalent with a clinical focus
Completion of a Post-Master of Nursing (NP Field) Diploma in Primary Health Care
Registration with the College of Nurses of Ontario within the RN (Extended Class)
Must have professional Liability Protection (RNAO or equivalent)
Current Basic Cardiac Life Support (BCLS) certification required
3-5 years of experience in an acute care setting preferred
Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders
Demonstrated commitment to equity, diversity, and inclusion (EDI) and experience advancing EDI objectives
Ability to promote professional education and advanced knowledge through research and evaluation/quality improvement activities related to the care of patients throughout the continuum
Demonstrated clinical leadership skills to organize and coordinate the care of patients in collaboration with their caregivers
Demonstrated time management skills with the ability and flexibility to prioritize multiple tasks in a fast-paced, results-oriented environment
Advanced nursing assessment skills and sound knowledge of clinical therapeutics related to Sickle Cell Disease
Effective patient/family counselling skills
Experience in acute and chronic pain preferred
Assets
Ability to speak another language
Experience caring for patients with Sickle Cell Disease
Paediatric experience
