POSITION PURPOSE:

The Team Leader directly supervises Direct Support Professional staff; assures proper implementation of Valiant Living policies and procedures; supervises and assists in the coordination and implementation of Individual Program Plans that includes programs in activities of daily living, community integration, socialization, and health management so the highest quality of life is afforded the people who are supported by Valiant Living. This is a non-exempt position.

RESPONSIBILITIES:

The Team Leader is responsible for the overall operation of one community residential setting.

SUPERVISOR: Residential Services Director (RSD) and/or Assistant Residential Director (ARD)

ESSENTIAL DUTIES:

Manages all tasks that support the direction, training, and strategic leadership of one home in the agency.

Provides training, direction, assessment of, discipline, and counseling to Direct Support Professionals and Direct Support Assistants in one home in the agency.

Assists in the development, implementation, and monitoring Individual Service Plans (ISP), to include staff training on implementation of plans for the people that are supported in one home in the agency.

Actively participates at individual meetings.

Develops and trains DSP and DSA skills concerning ISPs.

Monitors individual programs as outlined in the ISP.

Oversees and trains others to perform direct care duties for individuals living in the home, while assisting with hands on services as needed.

Responsible for handling complaints and resolving disputes or grievances from staff, clients or families in collaboration with the ARD and RSD.

Provides oversight for documentation, compliance, and procedures for medication management.

Completes assessments as required to oversee and monitor services.

Oversees organizational documentation according to regulatory requirements.

Able to use Microsoft Office programs, Therap documentation, email, and phone systems.

Advocates for the needs of the individuals residing within the home.

Manages individuals’ finances per established procedures.

Completes household operations, including but not limited to: cooking, cleaning, laundry, ordering groceries and supplies as necessary.

Identifies and responds to individual needs in emotional and social areas; however, intervention in these areas does not fall within the meaning of psychotherapy.

Performs tasks, which fall within Category II of infection control; may include exposure to blood, body fluids or tissue.

Ability to complete all ARSD training requirements within the required period of time.

Ability to maintain a flexible schedule, which may include working weekends and holidays.

Ability to be responsible for 24-hour on-call coverage and respond to issues as necessary.

Ability to lift 50 lbs. to 60 lbs. (depending on location of job location).

Ability to stand/walk for 75% of shift.

Lifting and moving, flexing, bending, crouching, kneeling.

Pushing wheelchairs and able to assist people with mobility.

Uses gait belts and lift apparatus safely, and able to train DSP and DSA for proper lift and gait belt use.

Possible exposure to bodily fluids.

Possible exposure to profanity, violent, and/or offensive behavior from individuals’ supported.

Passes a background check that meets Valiant Living, Inc. policy.

A negative result on a post-offer drug test.

Requirements

QUALIFICATIONS:

High School Diploma or equivalent required.

Must be 18 years of age.

Bachelor Degree in Human Services related field preferred.

Demonstrated proficiency in DSP Competencies.

2 years proven Supervisory/Management experience preferred.

Ability to complete mandatory orientation training, First Aid & CPR training, in-service training, and state Medication Administration certification.

Holds valid driver’s license and maintains a safe driving record in order to be insurable for all company vehicles.

Ability to be trained to drive Valiant Living owned vehicles within first sixty (60) days of employment.

Ability to maintain a flexible schedule, which includes working weekends and holidays.

Ability to manage multiple tasks.

Excellent problem solving, decision making and organizational skills.

Ability to read, write, and speak the English language adequately to respond to individual’s needs.

Ability to communicate effectively both verbally and in writing; listens accurately and responsively.

Responds to communication in timely manner and reviews e-mail daily.

Ability to see and hear in order to make assessments.

Ability to lift up to 50-60 pounds frequently, which includes lifting objects and/or transferring individuals.

Ability to sit, stand, stoop, bend, reach, and push/pull wheelchairs.

Management:

Is responsible for the training, supervision, evaluation and management of the Direct Support Professional staff.

Assure 24-hour coverage of the home and monitors services across multiple shifts.

Maintains staffing levels according to budgetary and safety guidelines per policies.

Assures that the appearance of the house and grounds meet standards for sanitation and infection control.

Individual Services:

Performs, oversees and trains direct care duties for individuals living in the home.

Assists in the development, implementation, and monitoring Individual Service Plan (ISP), to include staff training on implementation of plans. Actively participates at individual meetings.

Develops, trains, and monitors individual programs as outlined in the ISP.

Completes assessments as required to oversee and monitor services.

Completes documentation according to regulatory requirements.
