New opportunity in our primary care clinic for an RN Care Manager to provide care management services for patients living with mental health and substance use disorder with a high level of complex co-morbid physical healthcare issues.
The RN will lead and conduct care coordination/management with internal and external entities and collaborate with behavioral health and primary care staff internal and external to Southeast.
