Monday - Friday 8:30am-5:00pm

Interim HealthCare- Maine is currently seeking an experienced RN Case Manager to join our team in Oxford Hills, Maine. As an RN Case Manager, you will be responsible for coordinating and providing quality care to assigned patients in their homes or in healthcare facilities.Responsibilities:

Assessing patient needs and developing individualized care plans

Coordinating and collaborating with healthcare professionals to ensure effective patient care

Providing skilled nursing care, including administering medications, wound care, and monitoring vital signs

Educating patients and their families on health management and prevention

Advocating for patients' needs and rights

Documenting patient care and maintaining accurate medical recordsRequirements:

Valid RN license in the state of Maine

Prior experience as an RN Case Manager preferred

Excellent communication and interpersonal skills

Strong analytical and problem-solving abilities

Ability to work independently and as part of a team

Proficiency in electronic medical records (EMR) systemsBenefits:
