Description
Assists providers and other clinical staff and works independently to assess physical issues, vital signs, medication adherence, and plan issues, as well as medical problems, including psychological issues of the patient, and functions in a multi-disciplinary work environment by performing the following duties.
Essential Functions:
Completes nursing assessment of patient's condition consistent with age and condition; performs ongoing patient assessment including patient's health status, response to care, level of compliance and identifies variances from diagnosis
Works collaboratively with patient care team and family members to establish and implement plan of care including teaching and discharge plan
Shares observations and assessment outcomes with health care team and family to ensure communication and understanding of patient health status, progress and ongoing needs
Maintains and updates patient's medical records through concise, accurate and timely documentation
Participates in patient care rounds and patient care conferences as appropriate
Responsible for setting up and monitoring patients enrolled in disease specific home monitoring and communicating clinical information and changes to clinical plan between patient and provider/care team.
Develops and maintains patient registries based on disease, health condition and/or health prevention initiatives.
Identifies resources to integrate additional clinical practice guidelines to support continuity of care.
Ensures complete pre-visit planning for patients and coordinates referrals and outside services received or needed by patients.
As primary nursing care provider, supervises patient care activities of assigned team members, delegates activities appropriate to knowledge, skill and competency; provides timely feedback and review of care delivery to ensure maintenance of patient safety, infection control procedures and therapeutic environment
Assists with triage of patients, including triaging telephone calls and walk-in patients.
Provides orientation to the unit and hospital systems and processes to patients and family members to ensure familiarity with unit policies and requirements
Ensures compliance with all applicable medication, storage and documentation standards, regulations and requirements
Reviews and reinforces with patient/family ongoing education of basic technical procedures, activities and safety necessary for daily living
Participates in departmental orientation, on the job training and quality assurance programs/initiatives, a variety of department and hospital educational programs to maintain current skills and competency levels
Recognizes urgent/emergent patient and clinic situations and initiates emergency medical procedures.
Assists with obtaining lab results, medical tests, discharge summaries and other information within the hospital and other facilities and assists with arranging appointments, procedures and discharge planning.
Uses appropriate medical supplies and/or equipment for the assigned clinic, such as 12-lead EKGs, fetal monitor, pulse oximeter, etc. and collects patient specimens, including blood draw, if needed.
Processes lab specimens for transport or perform tests, such as waived tests, to obtain results.
Performs other duties as assigned Requirements
One year clinical nursing experience, preferred
Associates Degree in Nursing, BSN preferred
Current and active state unrestricted RN license, Arizona state license preferred
BLS certification required, ACLS and PALS preferred
