Overview Internal Medicine Primary care is where your path to wellness starts.
Your PCP is your first contact for general health questions, annual check-ups, vaccinations and illness.
It’s important to have a PCP who knows your medical history and can coordinate care with specialists when needed.
Your PCP is your medical home—a way of providing primary care that is patient centered and team based with a focus on coordination as well as quality and outcomes.
If you require more specialized care, your Catholic Health primary care provider will refer you to our vast network of experts or other resources across our continuum of care, such as home health, rehabilitation services, or hospice care.
But your PCP will always be there—as your medical home—to facilitate your health care needs at Catholic Health.
