Description
Summit Home Care and Hospice is searching for an experienced and motivated Licensed Practical Nurse (LPN) to join our Home Health team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.
Full-Time Licensed Practical Nurse (LPN):
Shift: Dayshift, Monday – Friday (40 Hours/Week)
Pay Rate: $28.00 - $34.00 per Hour
Location: Hamilton, Ohio
Expected Travel: 30 Mile Radius (Mileage Reimbursement, $0.48/Mile)
Responsibilities:
Provide direct patient care under the supervision of a registered nurse.
Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
Implement plan of care initiated by the registered nurse.
Provide accurate and timely documentation consistent with the plan of care.
Assesses and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
Use and/or prepare equipment and supplies effectively and efficiently adhering to aseptic technique.
Participate in personal and professional growth and development.
Perform other duties assigned by the registered nurse.
Qualifications
Qualifications:
Graduate of an accredited practical nurse or vocational nursing program.
Must hold a current license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
Current licensure in state and CPR certification.
Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements.
Recent TB test, required.
3 years of nursing experience, preferred.
Prior experience in community/home health and/or or medical/surgical, preferred.
Comply with accepted professional standards and practices.
Demonstrates good verbal and written communication, and organization skills.
Must be able to lift, position or transfer patients and lift supplies and equipment.
