We are currently offering $500.00 sign on bonuses! Bonuses are payable in two installments ($250.00 with first paycheck, and $250.00 after satisfactory completion of six-month probationary period).

What We’re Looking For:

Rock Creek Foundation is seeking a Full-time Day Program Counselor to join our team in Silver Spring, MD! As a Life Coach, you will be responsible for supporting people with disabilities achieve personal goals, as well as find meaningful work to increase their independence and social interconnectedness.

What You’ll Get:

Schedule: Tentative schedule of Monday through Friday 8:30am to 4:30pm

Hourly Rate Range: $20.00- $20.50

What You’ll Do:

Assesses individuals' social, emotional, financial, health, or other problems to develop person centered plan

Implements treatment plans with client through community based meaningful day supports and complete daily documentation to support services received and progress towards goal.

Complete monthly documentation towards individuals progress on implemented PCP goal

Complete Annual Assessment for each individual on caseload

Counsels individuals, assisting them to identify and resolve problems and make effective use of resources

Organizes and conducts meaningful educational, social, skill building, health & fitness, vocational community-based groups based on individuals interests and PCP goals

Works closely with families of individuals and provides appropriate clinical or community-based referrals for family members

Maintains working relationships with staff of other agencies and institutions, homes, and facilities, and acts as liaison between individuals and agency or institution

Develops and maintains contact with other agency staff and local service providers to promote coordination and integration of care

Helps individuals understand mental illness and how it affects their health and lifestyles

Encourages individuals to do things for themselves to retain feelings of independence and self-esteem

Prepares and maintains written case records within Electronic Medical Record, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required

Endeavors to place individual in gainful employment after acquisition of marketable skills

Resolve work related problems or difficulties, and assigns individual to simpler tasks when worker cannot perform assigned duties

Observes worker to ensure that work is performed according to prescribed methods and meets established standards

Maintains working relationships with staff of other agencies and institutions, homes, facilities, and acts as liaison between individuals and agency/ institution. Acts as liaison between individuals and their supervisor and co-workers

Helps individuals understand mental illness and how it affects their health, lifestyles, and employment

Determines suitable job or business consistent with applicant's desires, aptitudes, and physical, mental, and emotional limitations. Plans and arranges for applicant to study or train for job

Coordinate and implement general departmental projects

Responsible for ensuring staffing schedules and ratios are in compliance per individual’s plan/treatment plan

Back-up for absent employees, and shift rotations based on departmental and individual’s needs

Respond to and resolve customer complaints

Write and submit incident reports for department and submit in Electronic Medical Record within 24 hours of incident occurring per agency and State policy on reportable incidents

Oversees Quality Assurance measures at all assigned licensed sites

Adheres to all County, State DDA, MHA, and MBON policies and procedures

Ensures accurate and timely daily submission of individuals attendance in Electronic Medical Record per agency, State Medicaid, and DDA Policy and Procedures

Coordinate and implement general departmental projects

Responsible for ensuring staffing schedules and ratios are in compliance per individual’s plan/treatment plan

Attends mandatory meetings

Check’s work email at the beginning and end of each shift

Participates in and successfully completes all required trainings

Other duties as assigned

What We Require:

Must possess a valid driver’s license.

High School Diploma (or equivalent) and a minimum of two (2) years’ experience working with Adults diagnosed with varying mental health and intellectual disabilities.

Knowledge of Internet and Microsoft Office software

Must be able to work with “high-risk” consumers who have mental illnesses as well as developmental and physical disabilities.
