Advanced Practice House Calls – Nurse Practitioner or Physician Assistant for home visits
Position: Full-Time W2 Role
Will consider part-time
Location: North Houston and Surrounding Areas, TX
This position involves visiting patients in their homes for medical evaluation and management. No procedures.

Applicants must reside in the Woodlands, Spring, Tomball, or nearby areas.

Expected Start Date: Immediate or within 2 weeks of selection
Compensation and Benefits
Salary: $103,000–$110,000 per year plus help with transportation expenses and extra bonuses
Biweekly Pay: $3,961.5–$4,230.77 per pay period via direct deposit
Bonuses: Performance-based bonuses are available for extra notes.
Paid Time Off:
6 PTO/Sick days annually
8 paid holidays per year
1 week of paid vacation annually
2 paid bereavement days for immediate family
Insurance: Free malpractice insurance. Occurrence based $1,000,000 - $6,000,000. No tail coverage needed.
Salary Increase: 2% annually (conditions apply)
Position Details
Schedule: Monday–Friday, 9 AM–5 PM. No weekends, holidays, or on call.
Patient Load: 30–35 patients per week (6-7 patients daily). Most visits are follow-ups.
Office time: 2-3 hours paid for office time and patient load decreased on those days.
Visit Duration: 25–60 minutes per patient
EHR: Practice Fusion.
Driving Expectations:
Home visits within an optimized route, minimizing drive time.
Daily driving time is typically under 3 hours, starting from your first patient and ending at your home.
Responsibilities
Diagnose and manage acute, chronic, and episodic conditions
Prescribe medications and treatments
Perform thorough physical examinations
Develop and implement patient management plans
Facilitate referrals and coordinate care with home health agencies
Supervise and adjust home health services as needed
Support and Training
Providers are assigned a dedicated Medical Assistant to manage:
Route mapping
Confirming patient visits
Handling phone calls and messages
Processing and faxing orders
This support allows providers to focus on patient care.
Full training, shadowing, and ongoing clinical and administrative support are provided.
Preferred Skills and Experience
1 year of experience as an advanced practice provider (preferred, but not required)

Strong interpersonal and communication skills
Qualifications
Certified Nurse Practitioner or Physician Assistant license (required)
Flexible and able to manage daily driving within the specified schedule
