Community Physical was founded in 1986 by Bob Tripicchio, PT, MS, DSc, on the premise that people are the most important asset of any organization. CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. What You'll Do:

Deliver high-quality physical therapy services in a Skilled Nursing Facility (SNF) setting.

Collaborate with patients, families, and the rehab team to develop personalized treatment plans.

Maintain documentation and work closely with an interdisciplinary team to ensure the best patient outcomes.

Who We're Looking For:

Licensed Physical Therapist (PT) in Wisconsin.

Experience in Skilled Nursing Facilities (SNFs) preferred, but new grads are welcome!

Familiarity with Net Health is a plus-we're happy to train you!
