Clinic Hours:

Monday – Thursday, 8:00AM – 9:00PM

Friday 8:00AM – 8:00PM

Saturday 9:00AM – 5:00PM

SUMMARY

Family Service League is seeking a full-time Peer Specialist for the Family Recovery Center (FRC) in Riverhead. The Peer Specialist will be part of the multidisciplinary team at a co-occurring, outpatient, behavioral health program. The Peer Specialist will assist with the outreach support and coordination of care for clients, as well as linkages in the community that supports behavioral health recovery.

RESPONSIBILITIES

The Peer Specialist will provide outreach to clients in order to facilitate their engagement in services and offer additional support so they can engage in treatment and live successfully in the community.

Provide coaching and recovery support, including issues of self-advocacy, linkage to aftercare, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.

Provide concrete service delivery to clients, such as benefits and enrollments, as per individual needs.

The Peer Specialist will maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner, as per policy and procedure.

Coordinate care planning with other providers of services and resources to ensure goal directed, collaborative care, including care transitions.

Act as a resource to all team members on psychosocial and substance abuse issues and resources.

Provide telephonic, as well as face-to-face outreach.

Provide engagement and service planning during clients stay at the center and immediately following discharge.

Act as a linkage to community services, including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.

The Peer Specialist will monitor overall service delivery to clients to ensure coordination and continuity; advocate with service providers/resources as needed.

Provide crisis intervention and follow-up.

Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.

Abide by FSL’s Code of Ethics.

Establish and maintain positive working relationships with others both internally and externally.

Foster teamwork, work cooperatively and effectively with others to set goals, resolve problems and make decisions.

Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.

Set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information and activities.

All other duties as assigned.

QUALIFICATIONS

High School Diploma or equivalent required.

Peer Certification (CRPA), Provisional Certification (CRPA-P), or willingness to become certified within six months of hire required.

Relevant life experience required: personal experience living with a disability and/or social/emotional challenge, and willingness to draw upon this experience to serve as a role model.

Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Use Service Delivery System required.

Knowledge of Medicaid, Social Security and other entitlements preferred.

Proficient computer skills, including Microsoft Office and Electronic Health Records required.

Excellent interpersonal and verbal and written communication skills required.

Valid and clean NYS Driver’s License may be required as there is the possibility of driving for this position, but it’s not required.

Bilingual in Spanish preferred.

Some evening and weekend hours are required.

Ability to sit, stand, and stretch over periods of time.
