$42,000 - $45,000Responsibilities:
Develop and implement customized support plans for individuals based on their specific needs and goals.
Conduct regular assessments to track progress and make necessary adjustments to support plans.
Collaborate with a multidisciplinary team to ensure holistic care for each individual.
Provide guidance and support to individuals in accessing community resources and services.
Maintain accurate and up-to-date documentation of all interactions and progress notes.Qualifications:
3+ years of experience in customer service.
Proficiency in Microsoft Office Suite.
Strong verbal and written communication skills.
Ability to work independently and in a team environment.
Detail-oriented and highly organized.About Company
