Health Quest Home Care is seeking a compassionate Home Health RN - PRN Per Visit to provide exceptional care to patients in their homes. This per diem role allows for flexibility while working independently and as part of a coordinated team to support patient care, collaborating with physicians and families. Key Responsibilities:

Assess patients’ physical, emotional, and social needs, and create individualized care plans.

Monitor patient conditions, report changes, and communicate effectively with physicians.

Educate patients and families on disease management, wellness, and safety.

Provide treatments, perform screenings, and collect lab specimens as needed.

Ensure compliance with medical protocols and participate in ongoing education and quality improvement initiatives.

Qualifications:

Licensed Registered Nurse (RN) with home health experience.

Strong clinical skills and ability to work independently.

Must complete required continuing education and adhere to compliance standards.

Valid driver's license

Valid auto insurance

Working Conditions:

Physical effort: Ability to lift up to 50 lbs.

Exposure to typical home healthcare environments.
