Job Summary:
We are seeking a hardworking and highly skilledNurse Practitioner (NP) or Physician Assistant (PA) with wound care experience (certification preferred) to provide comprehensive, in-home and in-facility wound care services. The ideal candidate will be self-motivated, adaptable, and eager to be part of a rapidly growing healthcare organization that values innovation, efficiency, and excellence in patient care.
Key Responsibilities:
Provide advanced wound care treatments in home health, assisted living, nursing homes, long-term care, and skilled nursing facilities
Conduct comprehensive wound assessments and develop personalized treatment plans
Utilize evidence-based wound care techniques and implement advanced therapies
Collaborate with physicians, home health agencies, and facility staff to ensure seamless patient care
Maintain accurate documentation and compliance with healthcare regulations
Educate patients, caregivers, and facility staff on wound prevention and best practices
Adapt to a fast-paced, mobile healthcare environment while delivering high-quality, patient-centered care
Qualifications & Requirements:
Nurse Practitioner (NP) or Physician Assistant (PA) license active and unrestricted in the State(s) of Illinois
Wound care experience ideal; Wound Care Certification (WCC, CWCN, CWS, or similar) preferred
Ability to work independently and make critical patient care decisions
Strong interpersonal and communication skills to build relationships with patients and healthcare teams
Comfortable working in a mobile care setting with frequent travel to patient locations
Ability to adapt and thrive in a growing, fast-paced organization
