RN Field Case Manager – Home Health

Rockland, NYJob Summary:

We are seeking a dedicated RN Field Case Manager to join our growing home health team in Rockland, NY. In this role, you will provide high-quality patient care, coordinate with physicians, and supervise Home Health Aides (HHAs) while ensuring patients receive the best possible support in their homes.Key Responsibilities:

Perform home health admissions and reassessments per physician orders.

Coordinate and supervise HHAs in patient care delivery and discharge.

Develop and implement individualized care plans.

Observe, assess, and document patient progress.

Communicate with physicians and other healthcare providers regarding patient condition changes.

Educate patients and caregivers on disease processes, medications, and treatment plans.

Specialize in infusion care with provided training and ongoing skill development.Qualifications:

Active RN license in NY (Required).

Home health experience preferred, but we are willing to train nurses without OASIS experience.

Pediatric experience is highly preferred but not required.

Strong assessment and communication skills.Compensation & Benefits:

Full-Time: Nurses are required to complete 33 points per week (Start of Care = 2 points, Subsequent Visit = 1 point).

PRN Rates: $85 per revisit, $112 per Start of Care.

Mileage Reimbursement.
