Job Overview

We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. The ideal candidate will have a strong background in wound care management and be committed to providing high-quality patient care. This role involves assessing, diagnosing, and treating patients with various wound types, ensuring optimal healing and recovery. The Wound Care Nurse Practitioner will work collaboratively with other healthcare professionals to develop comprehensive care plans tailored to individual patient needs.

Duties

Conduct thorough assessments of patients with wounds, including history taking and physical examinations.

Develop and implement individualized treatment plans based on evidence-based practices.

Perform wound care procedures using aseptic techniques to prevent infection and promote healing.

Educate patients and their families on wound care management, prevention strategies, and overall health maintenance.

Collaborate with interdisciplinary teams, including physicians, nurses, and rehabilitation specialists, to ensure comprehensive patient care.

Document all patient interactions accurately in electronic health records (Cerner) to maintain continuity of care.

Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.

Provide support for patients with comorbidities such as dementia or those requiring specialized care in nursing home, senior homes, assisted living, or personal care homes.

Experience with Tele-Health is preferred but not required.

Requirements

Master’s degree in Nursing or equivalent qualification as a Nurse Practitioner.

Current state licensure as a Nurse Practitioner with certification in wound care Required.

Proficient experience in managing complex wounds.

Primary and Palliative home care experience.

Strong communication skills for effective patient education and collaboration with healthcare teams.

Ability to provide compassionate care for diverse populations, including elderly patients with dementia.

Previous experience in a nursing home or similar setting is required.
