Key Responsibilities
As an Advanced Practice Provider (APP), you will play a key role in leading the care team and delivering comprehensive primary healthcare to a panel of patients. You will provide medical oversight, expertise, and leadership to ensure the delivery of high-quality, cost-effective healthcare services.
Provide patient-centered, comprehensive primary care services consistent with a value-based care model
Conduct patient evaluations, diagnoses, and treatment plans based on evidence-based guidelines
Work collaboratively with physicians and other healthcare professionals to ensure continuity of care
Participate in quality improvement programs and contribute to the evaluation of practice performance metrics
Ensure compliance with privacy regulations, quality programs, and all HealthyU Family Medicine policies, procedures, and protocols
Utilize and implement new healthcare technologies, products, services, and programs to enhance patient care
Educate patients on preventive care, chronic disease management, and overall wellness
Maintain accurate and up-to-date patient documentation in the Electronic Health Record (EHR) system
Perform other duties as assignedRequirements:
Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
Valid state-issued license as an NP or PA (must be active by the first date of employment)
Current BLS (Basic Life Support) certification
Active DEA Registration
Ability to provide two professional references
One or more years of clinical experience in primary care
Experience providing longitudinal patient care (continuity of care)
Familiarity with managed healthcare systems, including quality assurance, quality improvement, risk management, population health, and disease management programs
Strong ability to work collaboratively within a multidisciplinary team
Excellent communication skills with both patients and colleagues
Self-motivated and results-driven approach to patient care
Preferred:
Bilingual (Spanish) strongly preferred
Experience with Athena EMR
