Salary: $25-$26
JOB SUMMARY: The KLEO CASE MANAGER is responsible for working with residents
to identify and address service needs including housing, benefits, health, legal, etc.
through referral or direct support under the direction of the Shift Supervisor at KLEOS
Shelter. K.L.E.O. Community Family Life Center believes in providing compassionate
and client-centered services that are culturally competent and linguistically appropriate
to all homeless residents.
KEY RESPONSIBILITIES:
ADMINISTRATIVE DUTIES
Intake new clients
Provide individual counseling for resident misconduct and connect to appropriate
supports
Works with residents and connects them to resources to ensure they arereceiving the support needed.
Shelter Intake/Exit
Shelter rules communication/enforcement
Misconduct conversations
Provides support to staff to de-escalate behavioral issues of residents
Host house meetings with their assigned floor/caseload
Prepares and reviews reports along with providing recommendations to leadership related to incidents involving residents
Support isolation needs for infectious diseases or general illness
Communication of City of Chicago and KLEO shelter policy changes in addition to utilization of databases.
Direct support to resettlement partners.
Internal reporting to ensure day/night shift is aware of incidents/identified needs
Assist as needed with special events as directed by supervisor
Ensure data collection is timely and accurate
May assist other shelters under the jurisdiction of KLEO.
Assists Residential Monitors and others as needed.
Able to perform the essential job functions consistent with the ADA, and otherfederal, state and local standards.
Excellent organizational and communication skills, both oral and written
Computer knowledge of MS Office or Google products.
Ability to work independently, prioritize tasks and meet deadlines with solid
organizational, analytical, and problem solving skills
Able to maintain regular, punctual attendance consistent with KLEO policy and procedures
MINIMUM REQUIREMENTS:
English/Spanish Fluency is preferred. Bilingual candidates must be able to fluently speak/write/read.French Creole preferred but not required
Previous experience working with homeless or at-risk community members
Knowledge and experience in providing services to at risk or members of the homeless community.
3 years of experience as a Case Manger or an Associates Degree in Psychology, Sociology, from an accredited college or university.
GENERAL PHYSICAL REQUIREMENTS
Exerting up to 25 pounds of force occasionally, and/or up to 25 pounds of force
frequently, and/or up to 25 pounds of force constantly to move objects.
