Registered Nurse - Home Health, acute care and/or Public health strongly preferred!

Are you a Registered Nurse looking for a new PRN opportunity? Abode Home Health is seeking a passionate, dedicated Home Health RN to join our team in Grand Junction, CO! You will make a difference in our patient's lives by providing safe patient care, deliver individualized nursing care plans to ensure our patients have an optimal care experience while working as part of an interdisciplinary team, supporting both patients and their families. Location: Grand Junction Branch

Coverage are: Grand Junction + as needed in Rifle, De Beque, Parachute, New Castle, Silt

Schedule: PRN, 10-15 hours/week

Complete initial and ongoing comprehensive assessments of patients and families

Initiate patient plan of care and reevaluate as necessary

Utilize health assessment data to determine nursing diagnosis

Administer medication and treatment as prescribed by the physician in the plan of care

Counsel the patient and family in meeting nursing and related needs

Complete and maintain accurate clinical notes

Communicate with the physician and participate in interdisciplinary group meetings to meet the medical needs of the patient

Ensure compliance with all state and federal legal and regulatory requirements

Additional responsibilities as assigned Qualifications

Graduate of an accredited nursing school with current licensure to practice in state of operation

1 year nursing experience required, 3+ years preferred

Experience in home health or a similar setting preferred

Commitment to clinical and documentation excellence
