We are seeking a dedicated and organized Home Health Intake Coordinator to join our team. As the Intake Coordinator, you will play a crucial role in the smooth functioning of our home health services by managing the intake process for new patients. This includes gathering essential information, coordinating with healthcare professionals, and ensuring a seamless transition for patients into our care.
Responsibilities:
Receive and process referrals for home health services
Coordinate with healthcare professionals, including physicians, nurses, and therapists, to gather necessary patient information and ensure timely initiation of services
Maintain accurate and up-to-date patient records, ensuring compliance with regulatory guidelines. Collect and verify patient information, insurance details, and medical history
Communicate effectively and provide excellent service to patients and their families, answer questions, and provide necessary information.
Monitor and track the progress of patients throughout their stay, and coordinate any necessary adjustments to the care plan.
Provide administrative support to the clinical team, including data entry, filing, and document management.
Participate in regular team meetings and training sessions to stay informed about industry best practices and company policies.
Qualifications:
Associates Degree
Experience in Home Health
Excellent communication and interpersonal abilities.
Strong organizational and communication skills.
Ability to prioritize and multitask in a fast-paced environment.
Proficient computer skills, including electronic health record (EHR) systems.
Compassionate, customer-focused approach
Proficiency in Microsoft Office (Word, Excel, & Powerpoint)
Knowledge of home health regulations and documentation requirements.
