Position Type: Full-time, Hybrid-Staff residing within the state of Ohio will be expected to attend meetings and events as required by their position or requested by their supervisor. These meetings may be held anywhere in Ohio, typically in Columbus.

Role Overview:

We are seeking a highly organized and detail-oriented professional to join our team as the Intake and Referral Coordinator for the Bridges Program. Reporting directly to the Program Administrator, this role is ideal for an individual who is passionate about supporting transition-age youth and ensuring they have seamless access to essential resources.

In this role, the Intake and Referral Coordinator will oversee the Bridges program statewide referral process, ensuring that referral sources and young adults receive timely engagement and clear guidance on eligibility requirements, intake procedures, and program services. They will also facilitate connections between young adults and the appropriate Bridges representatives, ensuring a smooth transition into the program. Additionally, the coordinator will track and analyze referral data, generating insightful reports on participant enrollment, referral trends, and provider caseload capacities. These data reports will support regional provider capacity planning, be incorporated into monthly and quarterly reports, and help streamline processes to optimize service delivery.

Key Responsibilities:

Collaborative Support: Manage the intake and referral process. Develop and maintain data and the referral platform with relevant intake and referral data to share. Ensure new program updates are effectively and consistently communicated to stakeholders during the referral process. Assist with onboarding and training new staff. Serve as the primary point of contact and subject matter expert. Manage a referral tracking system to determine capacity needs for each region. Maintain regular communication with key Bridges representatives and stakeholders.

Young Adult Support: Support young adults in transitioning to Bridges enrollment and assignment to the service team. Address questions and inquiries related to program services and the referral process. Provide valuable resource information to referral sources and young adults applying for the program. Facilitate connections between young adults and the appropriate Bridges representatives for assistance.

Process Improvement: Identify issues and opportunities within the current systems and suggest alternatives or enhancements to continually improve processes and procedures. Proactively identify referral trends, communicate findings to appropriate staff or teams, and provide insight and ideas for diagnosing issues and recommending changes or improvements.

Community Engagement: Attend and share valuable information about Bridges at conferences or other events hosted by community stakeholders.

Qualifications:

Associate’s degree and at least one year of office experience preferred

Excellent written and verbal communication skills

Highly organized; resourceful and proactive; pays close attention to detail

Strong time-management skills, with the ability to prioritize tasks

Ability to collaborate with multiple teams of people simultaneously

Ability to work independently and as a member of a team

High Proficiency with Microsoft Office 365 products (Outlook, SharePoint, Teams, Word, PowerPoint and Excel)

Familiarity with other databases, e.g., SACWIS and Apricot; preferred
