We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Queens, NY. The RN will collaborate with physicians, therapists, social workers, and other home health care professionals to deliver quality, patient-centered care.
Key Responsibilities:
Conduct comprehensive patient assessments in the home setting.
Develop individualized care plans based on patient needs and physician orders.
Provide direct nursing care including wound care, medication administration, IV therapy, and disease management education.
Monitor patient progress and revise care plans as needed.
Coordinate care with interdisciplinary team members.
Educate patients and caregivers on managing conditions and medications at home.
Document all care activities thoroughly in the EMR system in compliance with state and federal regulations.
Ensure compliance with agency policies and procedures, as well as Medicare/Medicaid guidelines.
Qualifications:
Valid New York State RN license required.
Associate's or Bachelor's Degree in Nursing.
Minimum 1 year of nursing experience; prior home care or community health experience strongly preferred.
